Transitions: Choices in Recovery

A Post-Care Survivorship Program of the Bennett Cancer Center at Stamford Hospital

Transitions: Choices in Recovery is a post-treatment survivorship program for patients and their families. The first of its kind in Fairfield County, Transitions is focused on survivorship.  Survivors are given the tools they need to regain energy, strength and confidence as they return to wellness. 

Among Transition's many benefits is the ability to help patients understand their appropriate medical follow-up.  An important aspect of this is the creation of an individual 'Survivorship Plan' for each patient, outlining the patient's diagnosis, treatment dates and recommended follow up.  This plan can then be shared with the survivor's primary care and any other appropriate physicians.  

Moving On

A special aspect of the Transitions program, "Moving On", has been designed to address the unique issues young adults face following a cancer diagnosis, and was made possible in part by a grant from the Connecticut Challenge.  "Moving On” is specifically designed for patients who are 40 years and under, are single, married or married with children. 

A special seminar series will be developed in which professionals will address practical issues, psychosocial needs, lifestyle changes and specific coping strategies.  Seminar topics might include: body image and sexual function, fertility and childbearing, dating and relationships, employment issues, and helping children cope with a parent's diagnosis.

The survivorship plan reinforces healthy living by including elements of the center's Integrative Medicine and Exercise and Nutrition programs. 

All participants in Transitions will have access to post care support programs including:

• A post care nutrition consultation and additional nutrition workshops
• Our post-treatment exercise program to help regain stamina, energy and strength
• A sampler of programs including Reiki, reflexology, yoga, t'ai chi and chair massage
• Various programs highlighting wellness, including stress management, art, music and sound therapy
• Educational programs to discuss specific issues and coping strategies
